Patient+ Privacy Policy

This privacy policy governs your use of the software application Patient+ for mobile devices that was created by momedx inc. Patient+ is an app that provides users with lab results, appointment scheduling, and other patient-facing tools.


User Provided Information

The Application does not directly collect user identifying information. Application registration is accomplished through an anonymized username provided by your health professional. Some features may require extra information, such as your name. This information is not saved in the Application itself and is safely guarded on HIPAA-compliant servers.

When you register and use the Application, activity in your account is associated with your medical record, as stored by your provider. Some user identifying information is stored on momedx servers in order to allow your health professional to utilize the system. This information is not transferred to or from the Application and is only accessible to authorized personnel. Such information is stored in a HIPAA-compliant manner.


Automatically Collected Information

In addition, the Application may collect certain information automatically, including, but not limited to, the type of mobile device you use, your mobile devices unique device ID, the IP address of your mobile device, your mobile operating system, the type of mobile Internet browsers you use, and information about the way you use the Application.

This Application does not collect precise information about the location of your mobile device.

Your information is never shared with third parties except for the limited uses cited below.

We may disclose User Provided and Automatically Collected Information:

• as required by law, such as to comply with a subpoena, or similar legal process;

• when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request;

• with our trusted services providers who work on our behalf, do not have an independent use of the information we disclose to them, and have agreed to adhere to the rules set forth in this privacy statement.
